Everything looks in order. When I see this error, it's typically related to using a TV as a monitor (which you are not doing), improperly installed video drivers (yours look fine), or a poor cabling connection. Can you make sure the DVI cable is securely pushed in on both the video card and the monitor? If there's an adapter in use, try using a cable not needing one.

If the cabling looks ok to you, I'll want to see the D3Debug.txt file next.

It's located in the main Diablo III game folder. Edit out your account name first though as it may appear in a few places in the file.

Looks like possible corrupted data related to Act3.

Try running the repair utility first as it's possible it might be able to fix it for you. If not, we'll go a little deeper and try a manual approach.

the repair utility is located under the Help tab on the game launcher.

Delete the following folder, \Diablo III\Data_D3\PC\MPQs\Cache, then run the repair tool again. It can take a while to run as it rebuilds the deleted info so please be patient and let it finish, then try the game again.
