When D3 first came out I bought it, beat it, and forgot it. Heard some coworkers discussing it, figured I didn't have something to currently play and I'd give it another go.
-Sapphire R9 270 Video Card, with newest driver
-Cosair SSD 75 Gigs for operating system and utilites, 500 Gig Toshiba External for Games and Programs in a 3.0 slot.
-8 GB of RAM
-Windows 7 OS 64-bit acquired from a guy with a parrot and eyepatch
-Offbrand Sceptre TV

I've tried reinstalling the game, updating driver, repairing game, optimizing via AMD gaming evolved, turning on v-sync.

System seems to easily handle it, but it stutters and speeds up framerate. I see this was an issue a few years back, but I cannot find a fix.

Hi Atropos!

Diablo III can be sensitive to anything that causes hard drive caching to be slow. If at all possible, clear some room on your SSD and move D3 over to it. If the problem vanishes, then you'll know the game was having an issue with the external drive.

If the issue doesn't vanish then I'd like to you to think back to when you were playing the game originally and try to remember if anything about your listed setup was different back then.

On a different subject, I'm not sure if you're serious about your Windows being pirated or not, but we do not provide support for our games on pirated OSes, and they frequently will not run well (or at all) on them. Just giving you a heads up.
