I'm in lower Alabama. I just recently got a new ISP, Time Warner, and have a basic Arris dg860 modem/router.

My problem is that I have a super fast internet connection (20mb down/5mb up) but my MS in-game is really fluctuating. I start at about 450-500, drop to 120-200, go up to 300-400, then it settles for a few under 100... I ran a pathtest, here's the results. No idea what it all means. Can anyone help me analyze?

It looks like the issue might be with your ISP's peering partner located at hop 9 of the pathping. When the connection reaches hop 9 it's experiencing a 29% data loss. That is a large amount of data to be losing, especially when it comes to online gaming. That data loss will cause for the connection to be weak by the time it reaches our game servers, which then leads to disconnects or increased latency.

I recommend that you contact your ISP so that they can look into this. They may even need to contact their peering partner, which seems to be TATA Communications, so that they can also look into this issue from their end.
