Okay first of all this is a two part issue caused by running medium or higher display DPI (text size) in Windows 7 (not sure about other versions of windows).

First issue is with the mouse, I've seen in some cases the mouse will not work at all or in my case it's working however it'll glitch out when I try to mouse over the UI. Now a fix for this would be to just lower the DPI but I cannot really, running 1920x1200 native resolution if I go on low everything will be too small and I'm not switching every time I want to play D3 that's stupid.

Another fix which leads into the second issue is running Fullscreen Window Mode however when I try to switch my whole screen turns black, can see and move mouse but nothing on screen, if I let it revert back everything is fine until I try to exit the game I get a black screen with no cursor and my whole computer seems to be locked up.

None of this was an issue months ago when I was playing steady so not sure what patch screwed things up but... Also my drivers are up to date and I've already confirmed it has to do with the display DPI and I'm seeing many reports of mouse issues and black screen issues.

Please look into this.

Try using a custom text size instead of using one of the larger preset values. For example, if I set it to 150%, the mouse doesn't work in-game when full screen, but a custom size of 149% works fine.
